You’ll need to fill in the NA5 form when applying for a Vietnamese visa, renewing it, or extending your temporary residence status. The form is in Vietnamese and English, and is required for all Vietnamese visa categories:

Follow these 5 steps to complete the Vietnam NA5 form:
1. Full name: in CAPITAL LETTERS. E.g.: TONY BLEAK
2. Sex: tick the box next to Male or Female.
3. Date of Birth: write as shown on your passport in the day – month – year format.
4. Nationality: fill in your nationality at birth, then your current nationality (if you have changed your nationality).
5. Occupation/Position: write your job title as shown on your work permit or labor contract.
6. Passport or International Travel Document number: fill in your passport number, type, issuing authority, and expiry date. Refer to the front page of your passport.
7. Date of the latest entry into Vietnam: fill in the date (in day – month – year format) and the name of the border gate. Refer to the latest Vietnam entry stamp you received on your passport.
8. Purpose of Entry: This may be similar to or different from the purpose of entry for your last visa in Vietnam.
9. Permitted to stay until: fill in your visa’s expiry date in the day – month – year format.
10. Temporary residential address in Vietnam: fill in your current address in Vietnam.
11. Contact telephone number/Email: use your own or your sponsor’s contact details.
1. Name of hosting organization: fill in the business name, address, phone number, and email of the organization (if you are sponsored by a relative, skip to the “Hosting family relative” section below.)
2. Hosting family relative:
1. To issue a visa: tick the box next to Single or Multiple entry and fill in the date – month – year of visa expiry. (If you are extending a current visa, skip to the next section.)
2. To extend the duration of temporary stay until: fill in the day – month – year (If you are applying for a new visa, do not fill in this section – return to the above section.)
3. Purpose: State in detail why you are applying for or extending your Vietnamese visa.
